Merging r292349:release_40
------------------------------------------------------------------------ r292349 | jlpeyton | 2017-01-18 01:02:21 -0600 (Wed, 18 Jan 2017) | 6 lines Fix memory error in case of reinit using kmp_set_defaults() for lock code. The lock tables were being reallocated if kmp_set_defaults() was called.
